MinnesotaCare is a managed care program that covers health care for people who do not have access to affordable health care coverage. Most enrollees pay a monthly premium, determined by a sliding-fee scale based on family size and income. There are exceptions for some military families and American Indians. Premiums are paid directly to the state.

You must first apply for MinnesotaCare coverage through your local Health and Human Services Department or online at www.MNsure.org. You can also apply for MinnesotaCare through the state MinnesotaCare office by calling 1-800-657-3672 to request an application.
If you are found to be eligible for MinnesotaCare and live in Itasca County, IMCare will be a health plan choice for you. You will receive a packet of information from the state office and you can enroll in IMCare at that time.
